Retrieving Tax Documents for Your Thrivent Annuities, Life Insurance, and Health Insurance

Tax forms are accessible online at Thrivent.com. Please note that Thrivent plans on generating another wave of tax documents around mid-February. If you do not see your tax documents the first time logging in, they may not be available just yet.

Go to Thrivent.com.  In the upper right-hand corner, click the login button.  Enter your user id (this is not your Thrivent id) and password.  You may be prompted to verify your identity by entering in a code sent to you via text, phone or email. The code will be sent to you by the method you selected.  Enter the code and continue to the Welcome page of your accounts.  Click on the link that says View Statements and Documents.  You will see a list of documents available to you.  Click on the tax forms.  You will be able to print and download tax forms.

If you have questions or need assistance logging into your Thrivent profile, please call Thrivent Customer Service at 1-800-847-4836.  When prompted, say “Representative” and the automated system will direct you to a customer service agent. The customer service representative will ask for your Thrivent ID or Contract Number and will further assist you with login questions, etc.
